Worth it if
Nature lovers, shrine visitors, photographers, ferry/day-trip people.
Skip it if
Travelers who are rushing Hiroshima as a short stop and cannot give Miyajima enough time.
A small island a 10-minute ferry from Hiroshima, where Itsukushima Shrine sits on wooden platforms over the tidal water and the large vermilion torii gate stands offshore. At high tide the gate appears to float; at low tide you walk across sand to it. There are also tame deer, forested hills, a hiking trail to Mount Misen's summit, and a main street of maple-leaf-flavoured things to eat.
The floating torii gate is genuinely one of Japan's finest views — famous for a real reason. But the island extends well beyond the photograph. Hiking to Mount Misen takes 2-3 hours and gives you the Seto Inland Sea spread below. Staying overnight, after the day visitors leave on the last ferry, gives you the shrine precincts at dawn with almost no one else there.
The torii gate appears to float at high tide, when water covers the tidal flats around it. At low tide the flats drain and you can walk out to the gate across wet sand, which is a completely different and less-photographed experience. Mid-tide gives you neither. Most visitors arrive with no idea which condition they will find and leave mildly confused about why the famous image does not match what is in front of them. Check the tide table before you go. Decide which version you actually want. The walk-out-to-it version is often better.

Why locals go
Miyajima has been a sacred site for over a thousand years and has long-standing religious significance beyond the famous photograph. The shrine itself, walking through the low corridors over tidal water, is architecturally interesting and not just visually so.
What visitors miss
The island quiets dramatically after 5pm when the day-trip crowds leave on the last regular ferries. An overnight stay or a late-afternoon visit gives you the shrine precincts and the village streets essentially to yourself in the early morning.

Practical tips
Check tide times at Miyajima (NHK World tide calendar or any Japan tide app). Plan arrival for high tide or low tide depending on whether you want the floating gate or the walk-to-it version.
The Mount Misen hike takes 90 minutes to the top. Strong views of the Seto Inland Sea. Cable car available if the full climb isn't practical.
Momiji manju — sweet cakes in maple leaf shape — are the local specialty. They're sold everywhere; the fresh ones from small bakeries are considerably better than the packaged versions.
